Why astigmatism has to be addressed
Removing a cataract does not remove corneal astigmatism. If it is left uncorrected, distance vision after surgery may still need spectacles, even when the surgery itself has gone perfectly.
Measurement comes before the decision
Optical biometry, corneal topography and keratometry are used to quantify the magnitude and axis of astigmatism, including the posterior corneal contribution. Repeat measurements are taken when readings disagree.
Alignment decides the result
A toric lens must sit on the intended axis. Even a small rotation reduces its effect, which is why marking, capsulotomy quality and stable lens positioning matter, and why a rotated lens occasionally needs repositioning.
Realistic outcome counselling
A toric monofocal lens targets clear distance vision; reading glasses are still usually required. Combining astigmatism correction with a multifocal, trifocal or EDOF design is possible in suitable eyes, and is discussed with its own trade-offs.
